HSH Notice of Privacy Practice
The Department of Homelessness and Supportive Housing Notice of Privacy Practice describes how personal information about clients may be used and shared by the City and County of San Francisco’s Department of Homelessness and Supportive Housing (HSH) and its provider organizations, and the rights clients have about their information.
Please note that if you receive funding by both Department of Public Health (DPH) and HSH, you will continue to use the DPH Notice of HIPAA Privacy Rights. Homeless Response System Release of Information
The Homeless Response System Release of Information explains how Client information is collected in the ONE System and used by or shared to housing and service providers (each, a “Partner Agency”, and collectively, the “Partner Agencies”), which includes community-based organizations and government agencies (such as the Department of Public Health or the Human Services Agency).
Human Service Agency Release of Information
The Human Service Agency Release of Information explains how the the Department of Homelessness and Supportive Housing’s Homeless Response System City staff and its contracted partners may access information on which, if any, public benefits clients receive from the Human Services Agency.
This information can also be found in the Release of Information electronic signature when creating a new client profile in the ONE system.
Where can I upload ROI forms?
Signed release of information forms can be uploaded into ONE by clicking on the files tab under client profile.
To add a new file, select Add File Link on upper right corner of files section.
Select Release of Information Category and appropriate file name. Upload file and save.
